Most goal-setting advice skips the most important step: breaking your goal into concrete weekly actions. Without that bridge, goals stay wishes. This free 2-page worksheet walks you through the full process from big goal to this week’s to-do list.

How to fill it out
Write one goal only. The worksheet is designed for single-goal focus. Make it specific and time-bound: not get fit, but run a 5K in under 30 minutes by September 1. Fill your why before your milestones. Your why determines how hard you push when things get hard.
Research from Dominican University found that people who wrote down their goals were 42 percent more likely to achieve them. Adding a weekly action plan raised that even further. This worksheet builds both habits into a single print.
Tips for using it every week
- Reprint the worksheet every Sunday for a fresh weekly action plan
- Keep your 90-day milestones visible by taping them to your monitor or planner cover
- Review your why every Monday morning before checking email
- Pair with the Habit Tracker to make your weekly actions non-negotiable
